Our office is open four days a week to provide you with appointments that are convenient for you.
Monday: 8am - 6pm
Tuesday: 7am-5pm
Wednesday: 7am-5pm
Thursday: 7am-5pm
*Emergency appointments are available daily.
Appointments
We gladly accept requests for appointments by phone or email and do our best to accommodate.
For your convenience, you may print and complete your health history form at home. Please remember to bring your completed form to your appointment.
Insurance & Billing
We accept most insurance plans and submit claims electronically for our patients. Co-payments and deductibles are collected at the time of service and we accept assignment of benefits from insurance companies. Estimated coverage is provided for larger cases from our data bank of insurance companies. Major credit cards are accepted.
We work with Care Credit, a convenient way to pay for treatment over an extended period of time. Ask us for details.
If your insurance provider is not listed or you do not have an insurance plan please do not hesitate to call us. Inquire about our In House Individual Dental Plan.
